Can any folk provide me the checklist for H1B visa application. Is I-129 W, a necessity?? Do I need to submit my credentials in original or a copy is accepted by the INS?

you mean VISA stamp in the embassy ?
for that you surely need the original plus the recen paystubs plus all your previous immigration documents stating your legal status in USA....

You MUST provide ORIGINAL I-797.
Recent Paystubs. And as posted earlier...proof of legal status since the day you enter US.For ex.(if applicable) I-20, Practical Training Card.
