Some of the ISPs with their own VSAT connection are selling phone cards in Nepal. Actually, they don't have phone cards in physical form but they will give you access number and code number and you can buy of any amount you like. Rates for US and Canada is Rs. 15 per minute and Japan is Rs. 30. Call to Europe should be somewhere between that. The quality is very good.

Since this is not legal as the VSAT agents were not supposed to provide voice communication, I can not give the contact address here. You can try some of the ISPs if you have contacts there. If not, then many roadside communication centers (popularly known as STD/ISD), specially with computer facility, will be an excellent source of information. The reason for putting those communication center in second priority is that they might take some extra profit from you.

There is also infotalk service to call overseas from Nepal. In order to use this service, either you have to buy their machine which costs about 10 thousand rupees or a card if you have computer. Then you need to have internet access. Their machine or card in a computer will access their network through internet. The rate for USA/CANADA is 10 cents per minute and of course their is the price of local telephone and internet connection. You can buy time for infotalk from any computer shops in Nepal.

Apparently, there is no way to call from USA to Nepal at this price. So friends, gone are the days for calling home at almost negligible rate. Now its the other way round. One problem is, you will feel that 15 rupees in Nepal is more expensive then 20 cents in US while the fact is the other way round.
