well, i am not a doc,, still ur problem looks similar to the one i had.
To fulfil the immigration formalities, i had to produce my vaccine history with proofs,, wasnt possible to have it living abroad. so consulted a doc,, he took my blood samples,, made the analysis for vaccines taken,,,and filled the vaccination record forms (it seems thats the way it works abroad when one is away from the country of vaccination). additionally by blood test reposts and chest-x ray film was attached to it and the purpose met.
m not sure if this gonna work for u,, but still can explore this option.
