Please let me know if I am missing anything and I questions too: $410 personal check Valid passport copy( do I have to include my previous expired one too) I-94 copy Visa copy (do I have to include this one) Previous TPS approval (It shows valid from 06/25/2018 to 06/24/2019 because now its automatically extended right?) Do I have to include OPT work permit? Do I have to include my I-20? can I pick a start date date like we do when we apply OPT? payable to U.S Department of Homeland Security Address: USCIS P.O. Sathi ho, I think I also need 2 passport size photos right? I am working is it a good idea or do I have to fill out employment information for TPS work permit or can I leave that section empty? And I chose the category a 12. Please let me know. It's urgent
